PrevCareMD is a service designed to help you provide a higher level of patient care, while maximizing the government of Ontario's annual Preventive Care bonuses. In short, PrevCareMD works by:
1. Identifying Patients who Require Interventions
PrevCareMD begins by identifying the patients who have had Preventive Care interventions. We help you access data sources including OHIP billing history, Ministry of Health Reports, Ontario Breast Screening Program lists, Cytobase, and others to get the most up-to-date information.
Then, we contact those patients for whom we have no information to determine whether they received interventions from other unrecorded sources (e.g. a flu shot at Shoppers Drug Mart). Contacting patients can occur in two ways: a demographically-targeted questionnaire, or friendly phone call.
2. Booking Interventions
After identifying which patients require interventions we will work with hospitals, clinics, labs as well as your office to book patients the interventions they require.
3. Tracking Performance to Target Levels
PrevCareMD will keep track of your patients on an ongoing basis and book their required interventions before they become due, ensuring the highest possible level of preventive care, and bonus attainment.
What are the key benefits of PrevCareMD?
PrevCareMD is designed to maximize up to $15,000 (as of April 1, 2008) of Cumulative Preventive Care bonuses, and increase the level of preventive care for all your patients, both those who come to see you regularly and those who do not.
How do I know if I am eligible for Preventive Care bonuses?
To be eligible for Preventive Care bonuses, you need to have a minimum of 650* patients enrolled in one of the following Enrollment Models: Family Heath Network (FHN), Family Health Organization (FHO), Family Health Group (FHG), Family Health Team (FHT), Comprehensive Care Model (CCM), Group Health Centre (GHC), Rural and Northern Physician Group (RNPGA), St. Joseph's Health Centre (SJHC), or South Eastern Academic Medical Organization (SEAMO).
*Unless you are a new graduate in your first year of practice with a FHG or CCM; in this case, you would need only 450 enrolled patients.
What is required of me and my staff?
Simply stated - almost nothing. PrevCareMD is extremely effective at collecting information and scheduling patients to receive the interventions they require.
Can PrevCareMD help me claim my Preventive Care Management Fees?
Yes, for FHN/FHO/FHT physicians, PrevCareMD will provide you with a list of patients contacted to effectively bill for Preventive Care Management Fees. This represents an additional bonus of $3,500 per year for an average practice with an enrollment base of 1,500 patients.
I already track my Preventive Care bonuses - why do I need PrevCareMD?
PrevCareMD not only ensures that enough of your patients receive the Preventive Care services for you to reach your bonuses, but we also ensure that the hard-to-reach patients are aware that they should receive these important services. Secondly, PrevCareMD offloads the large amount of work being performed by you and your staff in order to track these services on an ongoing basis.
